If you'd like to apply for a permit, please fill out one of
the following forms. You can send the form directly via email,
or print it out and fax it to 212.832.4557 ATTN: Donna Masly.
If faxing over an application, please call 212-832-4563 for fax
confirmation.
Please allow TEN business days to process all permits.
Please note: ALL permittees MUST produce a certificate of liability.
